Information collected and used

Forms collect your name, phone number, optional email, broad coverage category where requested, appointment details, and communication choices. These details are used to respond to your request, compare coverage options with you, arrange consultations, and maintain inquiry and consent records. Do not enter Social Security numbers, payment information, or detailed medical information in these forms.

Direct contact and service providers

I handle inquiries directly. I do not sell your lead or distribute it to other agents for their marketing. Submissions are stored in the Compass GoHighLevel CRM. Hosting, forms, scheduling, email, and communications providers process information to operate these services. Authorized platform administrators may have access as part of administering those services. This notice does not mean that only I can technically access data stored in these systems.

Calls and text messages

Submitting a review request asks me to contact you about that request. Text permissions are separate and optional. If text messaging is enabled, messages are sent according to your consent choices. Marketing consent is not a condition of purchase. Reply STOP to unsubscribe from texts or HELP for assistance. Contact me to change other communication preferences. SMS opt-in records are not provided to other businesses for their own marketing.

Website technology

The website and embedded CRM and scheduling tools may process technical information such as IP address, browser and device information, referring pages, and activity timestamps, and may use cookies or similar technology for operation and attribution. No advertising retargeting pixels have been activated by me for this launch. A QR scan alone does not identify an anonymous visitor. This website does not offer a separate response to browser Do Not Track signals; embedded providers apply their own technical practices.

Your requests and records

You may contact me to request access, correction, or deletion of information you submitted, or to withdraw a communication preference. Applicable insurance, consent, transaction, and other legal recordkeeping requirements may limit deletion. Please identify the request without sending sensitive medical or financial information by email. No online transmission or storage system can be guaranteed completely secure.
